1Missing or incorrect bleed - any design element that's meant to run to the edge of the page needs 0.125 inches of bleed added on all sides. Content placed exactly at the trim line without bleed gets a thin white sliver or gets cut off after printing.
Fix: add 0.125" bleed to any full-bleed element before export, and keep critical text 0.25" inside the trim line as a safe zone.
2Margins under KDP's minimum - interior margins need at least 0.25" on the outer edges, with a wider 0.375" on the gutter (binding) side to account for the page curve near the spine.
Fix: set gutter margin to 0.375" minimum, especially for higher page-count books where the binding curve is more pronounced.
3Cover sized for the wrong page count - spine width is calculated directly from page count and paper type, and a cover built before the interior is finalised often doesn't match once the actual page count changes during editing.
Fix: finalise interior page count first, then calculate spine width from that exact number - recalculate if page count changes even slightly.
4Using trademarked or copyrighted terms - including a trademarked brand name, a copyrighted character, or another publisher's series name in a title or interior, even in a generic-sounding low-content book, risks a takedown and can affect account standing.
Fix: use descriptive, generic category language ("Dark Academia Notebook" not a copyrighted franchise reference) - if in doubt, leave it out.

7Publishing without proofing a physical or accurate digital proof - on-screen PDF preview doesn't always catch margin or color issues that only show up in the final printed product, especially for interiors with shading or full-page graphics.
